If the course applicant is under 18 years of age we require a parent / guardian to agree that the applicant can attend the beginners course

Please note: Course attendees under the age of 16 must be accompanied by a parent / guardian

Do you have any medical conditions or special needs?

Please provide a brief description of any medical conditions or special needs that the course coaching team should be aware of e.g. visual impairment, hearing difficulty, mobility issues, please provide details below.

Any information provided will be treated in strict confidence and will only be used by the coaches to determine the most appropriate teaching methods and equipment to suit your personal needs.

If you prefer to discuss your needs with a coach please specify and a member of our coaching team will contact you.

Data Protection and Consents

High Elm Archers (HEA) do not share the information you have provided on this form with any third party organisations. HEA will retain this information for the duration of the course.

Data Protection legislation requires informed consent in order for us to use your data. To ensure we are compliant we are ask all applicant’s for the High Elm Archers beginner’s course to confirm their consent for High Elm Archers to use your data:

Declaration

I hereby apply to join the beginner’s course in archery with High Elm Archers Archery Club and enclose a deposit of £15.00* / the full course fee of £60* (* delete as appropriate).

Parental Consent

Required for all applicants under the age of 18. I the undersigned hereby consent to the above named person joining a beginner’s course in Archery with High Elm Archers. I confirm that I am the applicant’s Parent/Guardian.

Our preferred method of payment is by BACS (on-line banking) to High Elm Archers, Sort code 01-03-25 Acc no 81806531.
